MDAC is a cardio respiratory specialty clinic that services Thunder Bay and Region. The clinic does not accept self referrals. All tests require a referral on an MDAC requisition form signed by a Physician or Nurse Practitioner licensed to practice in Ontario.
MDAC offers all our diagnostic and consultation services under Ontario Health. You must present your valid Ontario Health Card or pay current rates to receive services.
MDAC offers all of our services for third party requests that are not covered by Ontario Health. Please contact our office directly to discuss services and fees.
